Accomplishes ER diversion of frail elderly at KGH and QHC-B (Belleville General Hospital) only . Individuals 75+ are screened for being at risk due to frailty when they present at the ER. All who screen positive are referred to 4CSS from the ER. 4CSS receives notification from ER and contacts clients to make an offer of 30 days of CSS services with all applicable client fees waived. If the client is interested, 4CSS forwards referral to the CSS Home Support agency in the client's geographical area.

An 86-year-old woman whose daughter lived with her had numerous recent trips to the ER. Eventually, surgery was scheduled for six weeks down the road, and she was to wait at home. The daughter told the 4CSS Co-ordinator that though her mother was receiving home care from the CCAC three times per week for personal care, "it's all the other things in a day that are wearing us out". She was very grateful of the offer of CSS services through easier+. Frozen Meals, Home Help and Foot Care services were provided. "These services came at the perfect time; these are what got us through".
