Facilitates earlier and more successful discharges of elderly and frail clients by providing escorted transportation home from the hospital, assistance settling in at home, and an offer of up to 7 days of CSS services with any all applicable client fees waived. Individuals - elderly or frail - are referred by any hospital in the SE LHIN to 4CSS for the HAL service package. 4CSS identifies a PSW provider for the assignment, coordinates the transportation and settling in services for the day of discharge, and notifies the geographically appropriate CSS Home Support agency regarding potential services required by the HAL client. The CSS agency then follows up with the client to offer CSS services. |
Home At Last Story:
An individual aged 85 was ready for discharge, relying on an adult child from transportation home from hospital. There were two major concerns: The child lived out of town and could therefore not come until the weekend and; the vehicle was not conducive to a comfortable ride. A referral to the HAL program secured transportation and support for this patient who as a result was not only discharged three days earlier, but went home with a plan that ensured that some other pressing concerns regarding the state of the house, due to the debilitating effects of the illness that had taken her by ambulance to the hospital were thoroughly and sensitively addressed. The HAL PSW who escorted this patient home and assisted with the basic settling in tasks, also undertook some clean-up tasks that would have humiliated the client had they fallen to her child, but which she also could not have done for herself.
